Why firefighters should track carcinogen exposures
Research from the International Association of Fire Fighters (IAFF) and the National Institute for Occupational Safety and Health (NIOSH) consistently shows that firefighters face significantly elevated rates of certain cancers compared to the general population. Products of combustion — benzene, formaldehyde, polycyclic aromatic hydrocarbons (PAHs), and heavy metals — are absorbed through skin contact with contaminated gear, inhalation during overhaul, and off-gassing from undecontaminated equipment.
Maintaining a personal exposure log creates a documented history critical for workers' compensation claims, presumptive cancer benefit eligibility, and long-term occupational health monitoring. Department records capture incident-level data but rarely include your individual role, PPE use, or specific decon practices. This personal log fills that gap.
How the risk score is calculated
Each incident is scored across four factors and classified as LOW, MEDIUM, or HIGH:
- Respiratory protection (highest weight): Full SCBA throughout scores lowest; no protection scores highest. Removing SCBA during overhaul or using only a half-mask scores in between.
- Role: Interior attack and overhaul involve the highest particle and chemical loads. Exterior, rehab, and command roles score lower.
- Burning materials: Synthetic and chemical materials (plastics, rubber, electronics, petroleum, foam, chemicals/unknown) score higher than natural materials. Multiple hazardous materials increase the score further.
- Decontamination: Full decon is the benchmark. Gross decon only — or no decon at all — adds to the risk score.
